In order for us to help you in a faster more complete manner we MUST have all the following paperwork filled out completely before you can receive your Medicare approved items. Please print this list and check to see that you have the following when you visit us: |

An original signed Certifying Physician’s Statement form(also known as the therapeutic shoe bill) from the primary doctor who is treating your diabetes. Medicare asks that this form be an original and not a fax. Also this can only be signed by a Medical Doctor (M.D.) or a Osteopath (D.O.) not a podiatrist. this form is available here at our store for pickup anytime at the hours listed above. |
| □ | A prescription for Diabetic Shoes and Insoles from your primary doctor or foot doctor(podiatrist, orthopedist). |
| □ | Depending on your
secondary insurance you may be required to pay a copay. Do you have means to pay for services rendered? |
| □ | Your Medicare card as well as any secondary insurance card or paper. As we will need to make a copy of each. |
