Insurance Coverage

Insurance coverage for pedorthic products and services varies widely.  We encourage patients to contact their insurance company to determine what type of coverage is provided and what deductibles and co-insurance (if any) may apply for prescription footwear and foot orthoses.  Keep in mind that your insurance policy may use certain fee schedules or allowances which may not coincide with our fees.  At the Foot Pain Relief Store all amounts charged are usual and customary for the products and services we offer.

The Foot Pain Relief Store accepts assignments For Medicare, Medicaid, Workers Compensation claims.  For Workers Compensation we ask that you bring in any pertinent information such as adjuster contact information, claim number and any previous medical references regarding your claim.

For clients with HMO or EPO plans, A referral by your primary care physician may be required which may delay delivery of orthopedic appliances. Foot Pain Relief Store will no longer accept HMO and EPO plans without prior authorization or precertification. We are working with many providers to join their networks to correct this issue. 


For Texas HMO patients:

Texas law protects your access to medical care. If you are not satisfied that your HMO is providing you with the medical services you need, you should write or call your HMO and complain. If your complaint is not resolved to your satisfaction, you can request an independent panel to review your complaint. THIS PANEL MUST BE MADE AVAILABLE TO YOU.
